Full-hour visits with real Doctors of Physical Therapy — plus specialists in women’s health, post-surgical recovery, and return-to-work.
Early & honest
We treat people early and tell you the truth about what they need — real care, no games, nothing minimized.

What it means for your people

  • Less pain, sooner. Treated early, right where they work — before a small problem becomes a big one.
  • No missed work. No half-days spent driving to and waiting at an offsite clinic — care happens on site, on the clock.
  • No financial worry. Onsite care with no bill, no copay, and no insurance to sort out.
  • Cared for and confident. They know exactly where to go, and that someone has their back.

The research is clear on early injury triage

Treat musculoskeletal injuries early, conservatively, and on site — before they escalate.

Onsite triage, doctor-led.

Real care delivered early, right on your floor — no referral, no insurance billing, no time lost offsite.

✓ OSHA-Approved First-Aid Triage

When an incident happens, we respond with the right treatment — right away

The moment something happens on your floor, a Doctor of Physical Therapy responds with the correct care for that injury — whether it calls for OSHA-recognized first aid or hands-on medical treatment. We’re trained to get your employee feeling better as fast as possible — and to keep them feeling safe, cared for, and confident the whole way through.

One pathway is OSHA-recognized first aid — real care OSHA classifies as non-recordable:

A full physical exam by a Doctor of Physical Therapy
Massage & soft-tissue work
Kinesio taping & non-rigid support
Guidance to reduce discomfort & modify activity

And when an injury needs more, we deliver full medical treatment on the spot — and carry it through our clinics if it needs formal PT. Either way, your employee gets the right care, immediately. The point isn’t the label — it’s the fastest path back to feeling good.

Quick Response Team

Not onsite the moment it happens? You’re still covered.

We’re in your backyard. If an incident happens outside our onsite hours, your team just messages our Quick Response Team — and we either get your employee seen the same day at our clinic, or we come to you. Real access, around your schedule.

Why a Doctor of Physical Therapy makes the call

Most work injuries are musculoskeletal — the strains, sprains, and back, shoulder, and knee complaints that happen on a floor. For those, the best clinical decision-maker you can have on site is a Doctor of Physical Therapy.

  • A real evaluation, on the spot. A hands-on physical exam and differential diagnosis — not a symptom checklist or a triage script.
  • Conservative care, started immediately. The approach research links to less imaging, fewer opioids, fewer surgeries, and faster return to work.
  • Knows when to escalate. Doctoral-level training to screen for red flags and recommend the right next step right away — whether it isn’t musculoskeletal, or it is musculoskeletal but needs expert care beyond us. (In Wisconsin, we recommend — your employee always chooses their provider.)
  • Knows the OSHA rules cold. Documents and classifies every incident accurately — first aid or medical treatment — so your records are honest, complete, and defensible.
  • Carries it through. If a case needs formal PT, the same team of doctors sees it all the way to full duty — no handoff, no dropped ball.

Real care. The right call. The fastest way back.

Jedd Wellenkotter, PT, DPT

“When a Doctor of Physical Therapy is making the call on your floor, your people get real clinical judgment — the right care, started early, and a clear read on anything that needs to go further. That’s the standard we bring to every partner.”

We care for your whole workforce

We understand how to keep employees healthy and happy — and it shows up in the everyday care, not just the work injuries. Here’s what a typical week looks like, and why so little of it ever becomes a claim.

  • ~80% — Everyday aches, pains & old injuries. Back, shoulder, and knee pain, chronic tightness, and flare-ups that affect how people feel at work — even when the issue didn’t start there. Catching these early is exactly what keeps a minor ache from escalating into a real injury on the job.
  • Post-surgical & elective surgery recovery. Shoulder, knee, hip, lumbar and more. We don’t replace their outpatient PT — we bridge the gap between the clinic and full job demands so they return safely and don’t backslide.
  • Women’s health support. Onsite pre- and post-partum care and return-to-work support for mothers. Consistently one of the most valued benefits we offer — and a real retention driver.
  • Ergonomics & movement coaching. Real-time adjustments to lifting, pushing, pulling, and repetitive tasks — often eliminating symptoms in a single visit, right at the workstation.
  • 5–10% — True work-related injuries. Still managed early to prevent escalation — but a much smaller share, because we’re addressing so much upstream before a small issue turns into an injury.

What onsite care includes

  • Care for any musculoskeletal issue — work-related or not; back, shoulder, knee, sprains, overuse.
  • Early symptom triage — first-aid-level care that keeps a minor issue from escalating into a serious injury, so people get the right treatment before a small problem becomes a big one.
  • Ergonomic & workstation coaching — assessments and fixes delivered right on the floor, tuned to the actual task and person.
  • Return-to-work guidance — job-specific modifications and progressions that keep people working safely instead of sidelined.
  • No friction for the employee — no referral, no insurance billing, no time lost driving offsite. Care happens on the clock, on site.
  • A full clinic minutes away — when a case genuinely needs formal PT, it doesn’t leave the family; same team, front to back.

Support for HR & Safety

  • One clear line of communication between HR, the employee, and supervisors on every work injury.
  • When HR can’t reach the medical team, we can. When an employee is being treated elsewhere and HR can’t get a hold of the provider, we get through — our directory and local relationships mean work restrictions come back accurate, consistent, and easy to act on.
  • Faster, smoother return-to-work — accurate restrictions and job-specific planning, without the usual back-and-forth.
  • Fewer offsite referrals — most issues resolved on site; the rest stay inside one coordinated team.
  • The area’s leading work-comp PT — every payer. We work with all workers’ comp carriers, so there’s no network barrier. If a case transfers to comp, it stays with the team best equipped in the region to manage comp rehab and get people back to full duty.

Where the ROI comes from

What’s good for your employees is great for your business.

Take real care of your people and the numbers follow — not from one line item, but from a stack of them working together:

Healthier, more engaged employees
Fewer OSHA recordables
Lower workers’ comp costs
Less lost work time
Fewer offsite visits
Fewer referrals & scripts (incl. reduced imaging & opioid exposure)
Higher retention

We measure the results for every onsite partner — visits, cases, and cost avoided. Ask us for documented numbers from a comparable manufacturer, sized to your facility.

What sets our program apart

Whole person
We treat everything, not just work injuries. ~80% of what we see isn’t work-related, and treating it early is exactly what prevents the claims.
Local, and we know the system
Real experience with OSHA recordkeeping and Wisconsin workers’ comp — plus 500+ providers across Southern Wisconsin in our directory. We know the rules, the carriers, and the people to reach.
Front to back
Onsite triage through work-comp rehab. We carry the case all the way through formal PT at our clinics, and if it transfers to comp you’re already with the area’s leading comp-rehab team.
PT-led
Doctors of Physical Therapy on the floor. Founded and run by practicing PTs — real clinical judgment, right on your floor.
Tech-enabled
We build our own clinical software: our documentation platform (Lune) plus purpose-built FCE and care-communication tools. Records, reporting, and communication run fast and clean — with the outcome data to prove the program’s value.

Full occupational-health capabilities

More than onsite hours — a complete industrial-rehab partner. Our team is OccuPro-certified across the services HR and Safety rely on most.

  • Post-Offer Employment Testing (POET) — job-matched physical screening after a conditional offer, so new hires can safely meet the real demands of the role. Cuts early injuries, turnover, and comp exposure before day one.
  • Functional Capacity Evaluations (FCE) — objective, defensible testing of what an employee can safely do, for return-to-work decisions, comp claims, and setting duty levels. Certified and Advanced-certified.
  • Work Hardening & Work Conditioning — structured, job-specific programs that rebuild the strength, endurance, and tolerance an injured worker needs to return to full duty.

About Forward PT

Forward PT is an independent, family-owned practice founded by husband-and-wife physical therapists Jedd and Kristen Wellenkotter, with clinics in Edgerton and Fitchburg, Wisconsin. No national chain, no call center — the owners are the clinicians, and we show up in person. We hold established onsite employer contracts across the Edgerton, Janesville, and Milton areas — proven relationships with HR and safety teams — and with our Fitchburg clinic we’re now bringing the same program to the Madison market. When you partner with a local business, you get a partner invested in this community the same way you are.

  • Fitchburg Clinic — minutes from Madison-area facilities
